Position Overview:
We are seeking a Licensed Master Social Worker (LMSW) to serve as a Nursing Home Surveyor|Complaint Investigator. In this vital role, you will conduct surveys, complaint investigations (including allegations of abuse|neglect), and compliance reviews at skilled nursing facilities. The ideal candidate will have strong regulatory knowledge, investigative skills, and a commitment to protecting the well-being of long-term care residents.

Qualifications:
~ Education: Masters Degree in Social Work (MSW)
~ Licensure: Active NYS LMSW (required)
~ Experience: 2+ years of clinical experience in a medical facility (nursing home preferred); prior surveillance|regulatory experience highly preferred
~ Technical Skills: Data entry in Federal|State reporting databases; SMQT Certification preferred
~ Soft Skills: Strong communication, investigative ability, attention to detail, regulatory compliance knowledge, ability to work independently

Key Responsibilities:
~ Conduct nursing home surveys and complaint investigations, including abuse|neglect cases
~ Draft Statements of Deficiencies (SOD) and reports using Principles of Documentation
~ Assess compliance with Plans of Correction
~ Enter data in Federal and State databases
~ Participate in State monitoring during Immediate Jeopardy situations
~ Testify in administrative hearings when required
~ Adhere to DOH|IPRO schedules, policies, and procedures
~ Travel to onsite nursing home locations as assigned
